   Employee who does not fix health problem

Hello professionals,

I have an employee with gout and has chosen to NOT take his medicine so his pain will hopefully lead to a medicaid disability. After discharge from the hospital for an unrelated problem he shows up for work with a cane and a bad limp. We sent him home 3 days to see a doctor to get the foot fixed. He shows up on day 2 to get some papers and says he has no money to see a doctor and will not borrow from family...so he is not going to a doctor. I see a workman's comp injury coming if we let him in the plant. He is the second to last hired for his position.

I want to terminate him. How would be best to approach this?
