How to increase workplace productivity?

All companies want to improve employee productivity and my limited understanding says that in order to improve employee productivity we must first measure the current state of their productivity. I am curious to know how to do this and if you have done this in any of your organizations.

Productivity is a ratio between Output and Input. By reducing Input, if you can produce same or more goods and or services, or by increasing little more input if you can producce much more output,then Productivity is increased in simple terms.
